There were work areas underneath the plywood and no indication of what load the plywood could safely hold.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. A) In the welding shop, there were oxygen and acetylene bottles stored next to a hand truck that were not secured from falling over.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. A) In the mowing shed, there was an outlet box that was missing its faceplate, exposing the wiring inside to damaging elements in the environment.

A) In room 203 there was a Bona Vista hot water heater and a Nespresso coffee maker both plugged into a 15 amp relocatable power tap (power strip) that was plugged into a second 15 amp relocatable power tap plugged into a 15 amp outlet on the wall. The combined current draw for the appliance was 17.9 amperes, which was more than the rating for the relocatable power taps and the wall outlet, but not more than the 20 amp branch circuit.
