29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) 1015 Sunset Street, Scranton, PA:  An employee  performing roofing work on a section of roof that had an eave height of thirty (30) feet was wearing a harness which did not have an attachment point in the center of the wearer's back, on or about September 2, 2022.  Avila's Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.502(d)(17), which was contained in OSHA inspection number 1568957, citation number 1, item number 1 and was affirmed as a final order on April 14, 2022, with respect to a workplace located at 1537 West St., Honesdale, PA.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1015 Sunset Street, Scranton, PA:  An employee accessing a residential porch roof on the front of the house was exposed to a fall of approximately fifteen (15) feet in that the extension ladder was not extended  at least three (3) feet above the upper lander surface and no grab poles were installed, on or about September 2, 2022.  b) 1015 Sunset Street, Scranton, PA:  An employee accessing a residential roof on the rear of the house was exposed to a fall of approximately fifteen (15) feet in that the extension ladder was not extended  at least three (3) feet above the upper lander surface and no grab poles were installed, on or about September 2, 2022.  Avila's Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection number 1568957, citation number 1, item number 2 and was affirmed as a final order on April 14, 2022, with respect to a workplace located at 1537 West St., Honesdale, PA.
