29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Employees engaged in spraying activities in the spray booth area that are required to use half facepiece cartridge respirators were exposed to respiratory hazards in that a written respiratory protection program had not been developed and implemented. Employees working with SHER-WOOD Water White Conversion Varnish are exposed to respiratory hazards to include: central nervous system (CNS) depression, respiratory tract irritation, coughing, nausea or vomiting, headache, drowsiness/fatigue, dizziness/vertigo and unconsciousness.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Employees engaged in glue laminate coating activities in the laminate room voluntarily using N95 respirators were exposed to respiratory hazards in that the information contained in Appendix D was not provided. If a respirator is used improperly or not kept clean, the respirator itself can become a hazard to the worker. Therefore, it is necessary to take certain precautions to be sure that the respirator itself does not present a hazard.

29 CFR 1910.134(m)(2):29 CFR 1910.134(m)(2): The employer did not establish a record of the qualitative or quantitative fit test administered to employees: Employees engaged in spraying activities in the spray booth area that are required to use half facepiece cartridge respirators are exposed to respiratory hazards in that the employer did not establish a record of the qualitative or quantitative fit test administered to employees. Fit testing is necessary to ensure the expected level of protection is provided by minimizing the total amount of contaminant leakage into the facepiece through the face seal. Respiratory hazards associated with exposure to SHER-WOOD Water White Conversion Varnish include: central nervous system (CNS) depression, respiratory tract irritation, coughing, nausea or vomiting, headache, drowsiness/fatigue, dizziness/vertigo and unconsciousness.
