29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) At 464 Abalone Ct, Fort Walton Beach, Florida: On or about September 6, 2022 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the working platform and the rails of the ladder did not extend at least three feet above the landing to provide safe access exposing his employees to a fall hazard of up to 11 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 464 Abalone Ct, Fort Walton Beach, Florida: On or about October 27, 2021 and at times prior; the employer allowed his employees to perform framing activities without fall protection exposing his employees to a fall hazard of 30 feet to the ground below. Panhandle Guest Design, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1561617, citation number 1, item number 2/b and was affirmed as a final order on 01/28/22, with respect to a workplace located at 214 A Angelfish Ave., Fort Walton Beach, Florida.
