29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, was not protected by protective helmets:  a) At the job site, located at 33732 Astoria Circle Wesley Chapel, FL; On or about 9/10/2022, the employer exposed employees to struck-by hazards, in that head protection was not utilized by employees while organizing materials and utilizing portable Makita circular saws on the ground surface and at the same time employees were performing roofing and framing operations approximately 10 feet directly above with unsecured pneumatic nail guns, lumber materials and roofing tools on the second level of the residence in construction.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) At the job site, located at 33732 Astoria Circle Wesley Chapel, FL; On or about 9/10/2022, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees operating pneumatic nail guns, hammers and Makita portal circular saws while performing roofing and framing works.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a)	At the job site, located at 33732 Astoria Circle Wesley Chapel, FL; On or about 9/10/2022, the employer exposed employees to fall hazards, in that fall protection was not utilized by  employees while performing roofing and framing work at approximately 10 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)	At the job site, located at 33732 Astoria Circle Wesley Chapel, FL; On or about 9/10/2022, the employer exposed employees to fall hazards, in that the portable ladder used to access the work area was not extended 3 feet above the upper landing surface.
