29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) 109 Orange Street (Exterior Front): An employees engaged in cleaning up construction debris on the ground level was exposed to a struck by hazard to the head from siding work taking place above. The employer did not ensure that protective helmets were provided to affected employees when there was potential danger of head injury from falling siding material, on or about 9/10/22.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) 109 Orange Street, Englewood, NJ (Exterior Front Facing Left Side): An employee engaged in the installation of siding material was exposed to a fall hazard of approximately 18 feet above the ground surface, and was not protected with a personal fall arrest system. The employer did not ensure that employees working from a ladder jack scaffold more than 10 feet above a lower level was protected from falling to the lower level by fall protection, on or about 9/10/22.
