29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. a) Jobsite located at 1459 30th street, Philadelphia, PA.: The employer did not ensure employees were provided with safe access to the roof in that the side rails of the 32 ft. Werner extension did not extend at least three feet above the upper landing surface. Condition was observed on or about September 10, 2022.
