29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.  On or about September 12, 2022, employees were framing a new home under construction without using hard hats, exposing employees to being struck by equipment and materials from above.

29 CFR 1926.102(a)(1): The employer did not  ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about September 13, 2022, employees used pneumatic nail guns to secure trusses of a residential home without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.  On or about September 12, 2022, and at times prior thereto, employees were framing a residential home under construction without the use of a fall protection system, exposing the employees to a hazard of falling approximately twenty feet to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  On or about September 12, 2022, and at times prior thereto, employees were installing trusses to a residential home under construction without the employer providing fall protection training, exposing employees to a fall hazard.
