29 CFR 1910.106(b)(6):Chemicals were not stored in a location where flammable vapors are not present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition include; open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: a) Fabrication Area, Welding area: On or about September 13, 2022, and at times prior, the employer exposed employees to fire hazards, in that a 5-gallon can of Xylene was improperly stored in close proximity to welding operations. b) Facility Building, Northeast Corner: On or about September 13, 2022, and at times prior, the employer exposed employees to fire hazards, in that three 53-gallon drum of paint metal primer were improperly stored in close proximity to welding operations.

Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles. a) Fabrication Area, Welding area: On or about September 13, 2022, and at times prior, the employer exposed employees to eye injury hazards, in that a welding area was not enclosed with appropriate screens or shields.

Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards The CGA Pamphlet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling." a) Fabrication Area, Southeast Gate: On or about September 13, 2022, and at times prior, the employer exposed employees to struck-by hazards while working in close proximity to eight unsecured propane tanks.
