29 CFR 1926.1431(h)(5)(i): The platform was not hoisted a few inches with the personnel and material/tools on board and inspected by a competent person to ensure it was secure and properly balanced immediately prior to each lift: The competent person inspections did not ensure that the personnel platform was secured and properly balanced, in the following instances: a) Grove Crane - The master link connecting the Boscaro platform to the crane hook was undersized and not properly balanced. b) Grove Crane - The hook safety latch was not pinned in the closed position, while the Boscaro platform was suspended.
