29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) Worksite located at 948 Narragansett Boulevard, Providence, RI 02905:  On or about September 15, 2022, an employee working on the ground cleaning roofing debris, was not wearing a protective helmet while overhead workers continued to strip the roof.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Worksite located at 948 Narragansett Boulevard, Providence, RI 02905:  On or about September 15, 2022, employees were not protected from fall hazards while working on a roof with heights of approximately twenty-two feet and four inches (22'4") above the lower level.  LTG Friends Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1384596, citation number 1, item number 1-a and was affirmed as a final order on July 2, 2019, with respect to a workplace located at 10 School Street, Johnston, RI 02919.
