29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location - 7 Broadway Street, Salisbury, MA On or about 9/16/2022, employees on a commercial roof were not protected from potential falls to a lower level while connecting to a roof bracket anchor was not installed per the manufacturer's instructions. The Luis Duchi Flores dba. LD Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(15), which was contained in OSHA inspection number 1504140, citation number 1, item number 2 and was affirmed as a final order on 03/29/2021, with respect to a workplace located at 128 Franklin St. Haverhill, MA.
