At a temporary worksite located at 1204 Orange Circle North in Orange Park, Florida 32073: On or about September 15, 2022, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On the front portion of roof of a temporary worksite located at 1204 Orange Circle North, Orange Park, Florida 32073: On or about September 15, 2022, the employer exposed two employees to fall hazards ranging from 6 to 9 1/2-foot, in that employees were using an extension ladder that did not extend 3 feet above the upper landing to egress and exit from the roof.
