(a) Rondeau Construction, Inc. on County Rd 64 & 6482 in Grand Lake, CO: On or about September 13, 2022, employees were exposed to cave-in hazards while repairing a sewer pipe in an unprotective trench that was approximately 5 feet deep. The competent person did not properly inspect the trench prior to employees entering the trench. Employees worked in a crouched position, with their heads below the top of the trench, and the trench had near vertical walls, visible cracks and fissures, and an undercut section.
