29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 6/24/2022 at a roof project located at 135 & 137 South Drive, Lackawanna, New York; employees were placing shingles on the roof of two houses. The roof had an 5 in 12 pitch and an eave height of approximately 18 feet. There was no fall protection in use. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On or about 6/24/2022 at a roof project located at 135 & 137 South Drive, Lackawanna, New York; employees were placing shingles on the roof of two houses. Employees were using an extension ladder to access the work area on the roof. The ladder extended 1 foot above the landing surface. NO ABATEMENT CERTIFICATION REQUIRED
