29 CFR 1926.501(b) (13):  The employer did not ensure that each employee engaged in residential roofing activities with unprotected sides and edges (6) feet or more above lower levels, was protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   a) September 15, 2022, employees engaged in residential roofing activities were exposed to falls of approximately 7 feet and approximately 18 feet.  G.S. Superior Construction, Inc. was previously cited for a violation of this occupational safety and health standard or an equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1586404 , citation number 1, item number 1/d and was affirmed as a final order on July 14, 2022, with respect to a workplace located at 2000 Ridge Road. Highland Park, IL. 60016.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       a) On or about September 15, 2022, employees were exposed to the hazard of falling where a portable extension ladder did not extend at least 3 feet above the landing surface of the roof deck.    G.S. Superior Construction, Inc. was previously cited for a violation of this occupational safety and health standard or an equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1586404, citation number 1, item number 2 and was affirmed as a final order on July 14, 2022, with respect to a workplace located at 2000 Ridge Road Highland Park, IL. 60016.   No abatement certification or documentation required for this violation.
