29 CFR 1926.451(g)(1)(vi): Employees performing overhand bricklaying operations from a supported scaffold were not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system: a) Jobsite on 2498 Coral St, Philadelphia, PA - On or about September 20, 2022, the employer did not protect employees from fall hazards on all open side and ends of the scaffold. Employees were performing overhand bricklaying operations from a supported scaffold that did not have guardrails on the sides of the work platform, thereby exposing employees to fall hazards of approximately 24 feet.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet above lower levels for a distance sufficient to protect employees below: a) Jobsite on 2498 Coral St, Philadelphia, PA - On or about September 20, 2022, the employer did not ensure toeboards were erected along the edge of platforms. Employees were conducting overhand bricklaying activities from the scaffold, approximately 24 feet above the ground level without toeboards, thereby exposing employees working below to struck-by hazards.
