29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) This violation occurred on or about September 13, 2022, where an employee operating and standing between the Baler machine V63HD and a Komatsu forklift. The employee used a wire cutter to cut the wire caught on the Baler machine. Lockout/tagout procedures was not used. The employee was exposed to caught-in-between hazard when the cardboard bale fell.
