29 CFR 1926.961(c)(2) Standard: Open disconnecting means. The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized were open. The employer rendered the means inoperable, unless its design did not so permit, and ensured that such means were tagged to indicate that employees were at work. AVD: On September 13, 2022, the employer applied its locks to all switches and other means through which known sources of electric energy may be supplied to an electrical substation prior to commencing maintenance work, but erroneously applied its lock to one of the switches that remained in the closed position, thereby exposing employees to electrical hazards.

29 CFR 1926.961(c)(6) Standard: Test for energized condition. After the applicable requirements in paragraphs (c)(1) through (c)(5) of this section had been followed and the system operator gave a clearance to the employee in charge, the employer did not ensure that the lines and equipment were deenergized by testing the lines and equipment to be worked with a device designed to detect voltage. AVD: On September 13, 2022 the employer tested the lines and equipment with a voltage proximity detector using the Live-Dead-Live method at an electrical substation prior to commencing maintenance work, but failed to test the equipment in one cell on the back side of the substation, which was erroneously believed to be outside the arc flash boundary, thereby exposing employees to electrical hazards.
