29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles: a) At the jobsite located at 5228 Fedora Cir Brooksville, FL 34602; on or about 9/20/22, the employer exposed employees to struck by hazards in that eye protection was not utilized while roofing and framing operations were being conducted with pneumatic nail guns and roofing tools.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. a) At the jobsite located at 5228 Fedora Cir Brooksville, FL 34602; on or about 9/20/22, the employer exposed employees to fall hazards in that fall protection was not utilized while roofing and framing operations were being conducted on a residential home approximately 25 feet in height. Cordova Construction Services Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1596933, citation number 1 item number 1 and was affirmed as a final order on 9/15/22, with respect to a workplace located at 1897-1903 Trailside Dr. Palm Harbor, FL 34683. Cordova Construction Services Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1385702, citation number 1 item number 1 and was affirmed as a final order on 9/24/19, with respect to a workplace located at 1099 Montgomery Bell Wesley Chapel, FL 33543.
