29 CFR  1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace.  a) On September 20, 2022, BCI Acrylic, Inc. did not ensure that employees were not using damaged personal protective equipment in the workplace. An employee utilized a Tyvek coverall while spraying foam that had the long sleeves cut off from the coverall. The Tyvek coverall is required to be used when spraying Isocyanate (MDI) "A" Component and Elastopor�H 11711R  in the Coating (Spray Booth) Department.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

BCI Acrylic, Inc.,  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.134(g)(1)(i)(A) which was contained in OSHA Inspection Number 1411875, Citation number 1, Item number 2a and was affirmed as a final order on October 22, 2019, with respect to the workplace located at 1800 Industrial Drive, Libertyville, IL, 60048.
