29 CFR  1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace.  a) On September 20, 2022, BCI Acrylic, Inc. did not ensure that employees were not using damaged personal protective equipment in the workplace. An employee utilized a Tyvek coverall while spraying foam that had the long sleeves cut off from the coverall. The Tyvek coverall is required to be used when spraying Isocyanate (MDI) "A" Component and Elastopor�H 11711R  in the Coating (Spray Booth) Department.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):  a) On September 20, 2022, BCI Acrylic, Inc. did not ensure that an employee who was required to use Honeywell 7700-30L half face tight-fitting negative pressure respirator when working with Isocyanate (MDI) "A" Component and Elastopor�H 11711R in the Coating (Spray Booth) Department passed the fit test before being required to wear a respirator.  No abatement certification or documentation is required for this item.

29 CFR  1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  (a) On September 20, 2022, BCI Acrylic, Inc. did not ensure that employees were annually fit tested prior to requiring the use of  Honeywell 7700-30L half face tight-fitting negative pressure respirators when working with Isocyanate (MDI) "A" Component and Elastopor�H 11711R in the Coating (Spray Booth) Department.  BCI Acrylic, Inc.,  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.134(f)(2) which was contained in OSHA Inspection Number 1411875, Citation number 1, Item number 1b and was affirmed as a final order on October 22, 2019, with respect to the workplace located at 1800 Industrial Drive, Libertyville, IL, 60048.  No abatement certification or documentation is required for this item.

29 CFR  1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  a) On September 20, 2022, BCI Acrylic Inc., allowed employees working with Isocyanate (MDI) "A" Component and Elastopor�H 11711R  in the Coating (Spray Booth) Department  to use Honeywell 7700-30L half face tight-fitting negative pressure respirators with facial hair that interfered with the seal between the facepiece and the face.  BCI Acrylic, Inc.,  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.134(g)(1)(i)(A) which was contained in OSHA Inspection Number 1411875, Citation number 1, Item number 2a and was affirmed as a final order on October 22, 2019, with respect to the workplace located at 1800 Industrial Drive, Libertyville, IL, 60048.
