Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. a) Warehouse area: On or about September 20, 2022 and at times prior, employees operated forklifts to move materials in obstructed aisles , and passageways which were not marked. Employees were exposed to struck-by hazards.

Interior, front and rear column baseplates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazard. b) Warehouse: On or about September 20, 2022, employees performed material handling operations using an industrial steal racking system for furniture merchandise stored on the shelving. Racking system cross braces were not maintained free from damage, exposing employees to struck-by and crushing hazard. c) Warehouse: On or about September 20, 2022, employees performed material handling operations using an industrial steal racking system for furniture merchandise stored on the shelving. No pallet support members were being utilized to support the furniture stored on pallets, exposing employees to struck-by and crushing hazard.
