This condition exposed employees to a delayed egress and fire hazard.

Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. a) Warehouse area: On or about March 15, 2023, and at times prior, employees operated forklifts to move materials in obstructed aisles and passageways which were not marked. Employees were exposed to struck-by hazards. Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. a) Warehouse: On or about March 15, 2023, material was stacked in tiers and at excess heights, was not secured exposing employees to struck-by and crush hazard. b) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal racking system for furniture merchandise stored on the shelving. Interior, front and rear column baseplates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazard. c) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal racking system for furniture merchandise stored on the shelving. Racking system cross braces were not maintained free from damage, exposing employees to struck-by and crushing hazard. d) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal racking system for furniture merchandise stored on the shelving. No pallet support members were being utilized to support the furniture stored on pallets, exposing employees to struck-by and crushing hazard.
