29 CFR  1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  a) Warehouse: On or about March 15, 2023, Southwest emergency exit was removed from employee access, however the old directional signs still pointed to that area which was now a dead end. This condition exposed employees to a delayed egress and fire hazard.

No materials or equipment may be placed, either permanently or temporarily, within the exit route.  a) Northeast emergency exit, on or about March 15, 2023, boxed merchandise was obstructing the exit route exposing employees to delayed egress and fire hazards.  b) Southeast emergency exit, on or about March 15, 2023, boxed merchandise was obstructing the exit route exposing employees to delayed egress and fire hazards.  AAFES Furniture store was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1623588, citation number 1, item number 1/a, and was affirmed as a final order on December 14, 2023, with respect to a workplace located at 3559 Neely Rd, Fort Wainwright, Alaska.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage were made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked.  a) Warehouse area:  On or about March 15, 2023, and at times prior, employees operated forklifts to move materials in obstructed aisles and passageways which were not marked. Employees were exposed to struck-by hazards.  AAFES Furniture store was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.176(a), which was contained in OSHA inspection number 1623588, citation number 1, item number 1/b and was affirmed as a final order on December 14, 2022, with respect to a workplace located at 3599 Neely Rd, Fort Wainwright, Alaska.

29 CFR 1910.176(b): Storage of material created a hazard. Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse.  a) Warehouse: On or about March 15, 2023, material was stacked in tiers and at excess heights, was not secured exposing employees to struck-by and crush hazard.  b) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal  racking system for furniture merchandise stored on the shelving. Interior, front and rear column baseplates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazard.  c) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal  racking system for furniture merchandise stored on the shelving. Racking system cross braces were not maintained free from damage, exposing employees to struck-by and crushing hazard.  d) Warehouse: On or about March 15, 2023, employees performed material handling operations using an industrial steal  racking system for furniture merchandise stored on the shelving. No pallet support members were being utilized to support the furniture stored on pallets, exposing employees to struck-by and crushing hazard.   AAFES Furniture Store was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.176(b), which was contained in OSHA inspection number 1623588, citation number 1, item number 3 and was affirmed as a final order on December 14, 2022,  with respect to a workplace located at 3599 Neely Rd., Fort Wainwright, Alaska.
