29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects was not protected by protective helmets:  a) At the jobsite located at 1 Wesley Chapel Blvd Wesley Chapel, FL 33544 - Sentosa Lexington Oaks Apartments; on or about 9/10/22, the employer exposed employees to struck by hazards in that head protection was not utilized while organizing materials and walking on the ground surface, while roofing and framing operations were conducted approximately 30 feet in height directly above with unsecured pneumatic nail guns, lumber materials and roofing tools on the roof top.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) At the jobsite located at 1 Wesley Chapel Blvd Wesley Chapel, FL 33544 - Sentosa Lexington Oaks Apartments; on or about 9/10/22, the employer exposed employees to struck by hazards in that eye protection was not utilized while roofing and framing operations were being conducted with pneumatic nail guns and roofing tools.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was  not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a) At the jobsite located at 1 Wesley Chapel Blvd Wesley Chapel, FL 33544 - Sentosa Lexington Oaks Apartments; on or about 9/10/22, the employer exposed employees to fall hazards in that fall protection was not utilized while roofing and framing operations were being conducted on a residential apartment building approximately 30 feet in height.
