Location: Resin storage area On or about August 18, 2022, the employer did not ensure that motors, pumps, and electrical equipment in class 1, division 2 areas were rated to prevent the ignition of flammable liquids or vapors.

29 CFR 1910.160(b)(5): Hazard warning or caution signs were not posted at the entrance to, and inside of, areas protected by fixed extinguishing systems which use agents in concentrations known to be hazardous to employee safety and health: Location: Mezzanine mixing vats On or about August 18, 2022, employees were not warned by caution signs of the hazard present from the carbon dioxide fire suppression system above the mixing vats. The system had a design concentration of 4%, presenting a hazardous atmosphere during discharge.
