29 CFR  1926.51(a)(1):An adequate supply of potable water was not provided in all places of employment:  a.  On the roof at a residence, located at 34 Marsh Dunes, St. Johns, FL:  On or about 9/21/2022, the employer exposed employees to health hazards such as stroke and other heat-related illnesses, in that the employer did not provide drinking water while employees were working on the residential roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a.  On the roof at a residence, located at 34 Marsh Dunes, St. Johns, FL:  On or about 9/21/2022, the employer exposed employees to fall hazards ranging from 6 to 13 feet, in that fall protection training was not provided prior to the employees performing roofing work.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.  On the roof at a residence, located at 34 Marsh Dunes, St. Johns, FL:  On or about 9/21/2022, the employer exposed employees to fall hazards ranging from 6 to 13 feet, in that the side rails of a ladder were not extended at least 3 feet above the roof while employees were accessing the roof.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a.  On the roof at a residence, located at 34 Marsh Dunes, St. Johns, FL:  On or about 9/21/2022, the employer exposed employees to fall hazards ranging from 6 to 13 feet, in that fall protection, such as a harness with a lanyard attached to the roof  was not utilized, while performing roofing work.  The CG Roofing Group, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1474667, citation number 1, item number 1 and was affirmed as a final order on August 31,  2020, with respect to a workplace located at 1600 Wedgewood Drive, Gurnee, IL 60031.  The CG Roofing Group, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1536421, citation number 1, item number 1 and was affirmed as a final order on September 17, 2021, with respect to a workplace located at 830 Oak Street, Jacksonville, FL.
