29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a) On September 17, 2022, an employee was exposed to machine hazards associated with moving parts during the Set-up and unjamming of parts for the Chicago Dreis & Krump Mfg. Co. press brake, Model 810-C, Serial No. L-14487. The employer failed to ensure that employees implement energy control application steps as the press brake was not shut down or turned off to perform servicing work such as Set-up or unjamming [per the 1910.147(d)(2)requirements.] As a result the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation or residual energy [1910.147(d)(5)(i), and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.   No abatement certification or documentation is required for this item.
