Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   415 Silas Deane Hwy, Wethersfield, CT, office building roofing project: On or about September 21, 2022, the employer did not ensure appropriate fall protection was implemented for employees while performing commercial roof repair activities on a flat roof that was greater than 50 feet wide.  The employees were exposed to a fall of approximately 40 feet.
