29 CFR 1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about September 26, 2022, at a new commercial construction jobsite in Montgomery, Texas, an employees was exposed to a fall hazard when performing work from a wooden pallet placed on the forks of a JB Ingersoll Rand forklift, without the forklift being designed for such use.

29 CFR 1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about September 26, 2022, on a commercial building being constructed, employees were exposed to a fall hazard exceeding fifteen feet when performing steel erection activities without a fall protection system.
