29 CFR 1910.178(n)(8): Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner: On or about September 19, 2022, the PIT Reach Truck 79 was not operated at a speed that would permit it to be brought to a stop in a safe manner as required. The employer failed to ensure the work floor of the finished goods cooler (palletizing) was evaluated and slower speeds mandated during conditions where the floor was icy/wet due to construction being done in the room causing severe condensation.
