(a) Shop area at 1655 Elmwood Avenue, Suite 43, Cranston, RI 02910: On or about September 29, 2022, the employer did not determine if the voluntary use of a respirator created hazards including, but not limited to, cardiac and/or pulmonary disorders that could be aggravated by respirator use, wearing of a dirty respirator that can cause dermatitis or ingestion of a hazardous chemical, or sharing of a respirator that leads to transmittal of disease, for employees that are provided with or allowed to wear N-95 and elastomeric respirators.
