29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about September 27, 2022, employees were installing roofing material using Hitachi nail guns at a new residential home located at 3209 Rosewood, Red Oak, TX.  Employee(s) working from the ground and passing roofing materials up to the upper level was exposed to head injuries where overhead hazard exists.

Employee(s) were installing roofing material using Hitachi nail guns were exposed to eye injuries by not wearing safety glasses.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about September 27, 2022, at a new residential home located at 3209 Rosewood, Red Oak, TX. Employee(s) were installing roofing material on a new residential home and using a 16-feet extension ladder on the front side of the home that did not extend 3-feet above the landing of the roof.  The employees were exposed to a fall hazard.

The  employer did not develop or provide fall protection training to the employee(s) engaged in framing work to recognize and eliminate fall hazards when working over 6 feet and utilizing extension ladders.
