29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  4 East Meadow Road, Westport CT: On or about September 28, 2022, the employer did not ensure two workers on a roof more than 20 feet above the ground were protected by a personal fall arrest system when one worker fell and suffered a broken pelvis and arm.

29 CFR  1904.32(a)(1): At the end of each calendar year the employer did not review its OSHA 300 log to verify that the entries were complete and accurate.  On or about March 22, 2023, the employer had not reviewed the 2022 OSHA 300 log of Work-Related Injuries and Illnesses for completeness and accuracy and the incorrect data was transferred to the 2022 OSHA Form 300A Annual Summary in that:  a) On or about September 28, 2022, an employee sustained a fractured pelvis and fractured left arm after falling from a roof.  The injury would be appropriately captured with a check or an "x" in Column H (Cases with Days Away). b) On or about March 22, 2023, there were two cases (Z01742304 and Z01750181) with Days Away From Work (Column H) but the page total for number of cases with Days Away From Work was 49.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Establishment: On or about September 28, 2022, the employer did not keep any written certification records of fall hazard training for the two workers exposed to fall hazards over 20 feet above the ground where one worker fell and suffered broken bones.
