29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) 8 Circuit Hydrogen System: On October 4, 2022 and times prior, employees were not provided or required to wear flame retardant clothing (FRC) while working in areas designated as Class I, Division II within this hydrogen system. Employees are exposed to potential flash fire hazards while working on equipment in these areas which may contain flammable hydrogen gas.
