BRIAN & PAT GAVLINSKI DBA GAVLINSKI ROOFING & CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.102(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1423043, CITATION NUMBER 1, ITEM NUMBER  1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 2, 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 1722 W BEL AIRE AVENUE, PEORIA ILLINOIS 61614.

29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:  On or about October 5, 2022, employees were exposed to a fall hazard up to 12 feet while installing roofing material on a residential roof and a suitable fall protection system was not used.  BRIAN & PAT GAVLINSKI DBA GAVLINSKI ROOFING & CONSTRUCTION  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1425841, CITATION NUMBER 1, ITEM NUMBER  1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 2, 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 5231 N RONALD ROAD, PEORIA, ILLINOIS 61614. 29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:  On or about October 5, 2022, the employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employee's name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
