29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer is failing to protect employees from crush-by and struck-by hazards associated with the unexpected release of stored energy. This was most recently documented on or about October 05, 2022, at the job site located at 1298 Bison Rd, Hays, KS 67601. Employees engaged in maintenance activities underneath a tag axle were exposed to crush-by and struck-by hazards in that the employer did not establish an energy control program.

The employer did not ensure that oxygen cylinders were stored separately from highly combustible materials. Oxygen cylinders were being stored next to acetylene cylinders.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces: Employees are exposed to electrical shock hazards. Employees engaged in production activities in the maintenance shop were exposed to electrical shock hazards in that flexible cords used to power equipment while attached to building surfaces. Flexible cords powering drop lights and test equipment were being used while permanently attached to the building structure above and adjacent to each maintenance bay where heavy commercial vehicles are being operated.
