The hook was kept in service and not replaced.

29 CFR 1910.179(l)(3)(iii)(a): Crane hooks showing defects described in paragraph (j)(2)(iii) of this section were not discarded: (a) Production Work Area: On or about October 6, 2022, a 15 ton, Bridge Crane was utilized to lift and move steel pipe. The crane hook provided had visual defects to include "stretched" throat opening and remained in service.
