29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) Warehouse: On or about October 7, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not ensure that the propane tank on "Yale" forklift, model GLP030VXNEAE084 (serial number D810V03679T) was secured to the vehicle while employees operated the forklift in the performance of assigned duties.
