29 CFR 1910.180(h)(3)(i)(b): The employer did not assure that the load was well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches. On or about October 9, 2022, prior to lifting a tank for a second time, the load and slings were not checked to ensure the load was secured and properly balanced in the sling. This exposed the employees to struck-by and crushed-by hazards.
