29 CFR 1926.651(h)(1):Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. (a) On or about October 12, 2022 at the northeast intersection of Broadsmore Drive & Randall Road, Algonquin, Illinois, the employer did not ensure that employees were protected against the hazards posed by water accumulation. Employees were replacing a water main valve and water had accumulated in the trench from rain fall and pipe discharge. The pump on site was not adequate to remove the water.

29 CFR 1926.651(i)(3):Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures. (a) On or about October 12, 2022 at the northeast intersection of Broadsmore Drive & Randall Road, Algonquin, Illinois, the employer did not ensure that employees were protected against the hazards posed by possible collapse of pavements. Employees were installing a water main and a valve after excavating the area with hand tools and powered industrial vehicles. The employer did not provide any support to the pavement, including but not limited to Broadsmore Drive. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
