(a) Location - 46 North Avenue, Haverhill, MA On or about 10/06/2022, John Lanzafame dba All Under One Roof, as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. Employees of subcontractor MGG Construction, Inc were exposed to fall hazards of greater than 6 feet above a lower level.
