29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident:  a) Warehouse- On or about November 16, 2022, the employer did not conduct refresher training, including an evaluation of the forklift operator's knowledge and skill on relevant topics, following an incident where the stand up forklift operator struck the overhead busbar duct #7.  b) Warehouse- On or about December 17, 2022, the employer did not conduct refresher training, including an evaluation of the forklift operator's knowledge and skill on relevant topics, following an incident where the stand up forklift operator struck the overhead busbar.  c) Warehouse- On or about January 12, 2023, the employer did not conduct refresher training, including an evaluation of the forklift operator's knowledge and skill on relevant topics, following an incident where the stand up forklift operator was stacking two pallets, when the skid broke and the top pallet landed on the forklift overhead guard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
