1910.176(b):  Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  a) On October 6, 2022, at or near the Schokowsky 44-12H in Killdeer, ND, while offloading the rotating head from the rig floor during rigging down operations of H&P Rig 259,  an employee was struck by the rotating head and caught between the rotating head and the lower pipe guide (goal post) of the pipe delivery system.  Abatement Note: Among other methods, feasible and acceptable means of abatement include but are not limited to: 1) Conduct risk assessments in accordance with API RP 54, 6.11.1 prior to commencing rig-down operations. 2)  Ensure floor hands and operators are trained to recognize struck-by, caught-in-between, pinch points, and body position hazards and to take effective measures to minimize those hazards. 3) Ensure tag lines are in use to control loads and prevent employees from being struck by or caught in between the load and other objects.
