29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.  On or about October 14, 2022, an employee was connecting a hose to a Craftsman CMEC6150 air compressor without using hard hats when working below other employees who were installing shingles on a roof, exposing the employee to being struck-by equipment and materials from above.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  On or about October 14, 2022, an employee used a pneumatic nail gun to secure shingles to a roof without wearing appropriate eye protection, exposing the employee to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.  On or about October 14, 2022, employees were installing shingles to the roof of a residential home under new construction and working approximately 20 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:   On or about October 14, 2022, an employee accessed the roof of a residential home under new construction by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 10 feet to the ground below.
