29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about October 14, 2022, a lockout tagout program was not established while  performing serving or maintenance on an American Hydraulic Scissor lift. This condition exposed employees to struck-by or amputation.  b) On or about October 14, 2022, a lockout tagout program was not established while  performing serving or maintenance on an AO Smith water heater. This condition exposed employees to electrical hazard or  fire hazard.  c) On or about October 14, 2022, a lockout tagout program was not established while  performing serving or maintenance on an American Webster Compressor. This condition exposed employees to electrical hazard.

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.  a) Carpentry Shop: On or about October 14, 2022 the exit sign for emergency exits did not meet the standard in that it was faded red sticker on the door.  b) Vehicle Maintenance Bay: On or about October 14, 2022 the exit sign for emergency exits  did not meet the standard in that it was faded red sticker on the door.  c) Equipment Maintenance Bay: On or about October 14, 2022 the exit sign for emergency exits  did not meet the standard in that it was spray painted red on the door.
