29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.    a. Outside wall of drive through booth: An employee was not wearing eye protection when mixing and applying Quikrete Mortar Mix containing Portland cement to the surface of the wall with hand tools. Employee was exposed to chemical hazards.  b. Drive through curb: An employee was not wearing eye protection when mixing and applying Quikrete Mortar Mix containing Portland cement to the surface of the curb with hand tools. Employee was exposed to chemical hazards.

29 CFR  1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical (Construction Reference: 1926.59):  Agana McDonalds Job Site: Copies of the Safety Data Sheets (SDS) were not maintained onsite for hazardous chemicals to include Mortar Mix Quikrete No. 1102 and Concrete Bonding Adhesive that were in use at the jobsite. Employees were exposed to chemical hazards.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a. Outside wall of drive through booth: An employee was not wearing gloves when mixing and applying Quikrete Mortar Mix containing Portland cement to the surface of the wall with hand tools. Employee were exposed to chemical hazards.  b. Drive through curb: An employee was not wearing gloves when mixing and applying Quikrete Mortar Mix containing Portland cement to the surface of the curb with hand tools. Employee were exposed to chemical hazards.
