On or about October 14, 2022 and at times prior to, an employee was performing metal fabrication operations while using a leaf spring clip press machine without guards installed, exposing the employee to caught-in hazards.

29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used, where run through holes in walls, ceilings, or floors. On or about October 14, 2022 and at times prior, employees were exposed to electric shock hazards when a flexible cord ran through the wall of the production supervisor's office to provide power to personal protective equipment (PPE) vending machines located outside of the office.
