29 CFR  1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  Electrical Panel, behind the back door: An employee working on live electrical parts at 120 volts was not provided with voltage rated insulating rubber gloves.  Employee was exposed to electrical hazards.

29 CFR  1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  Electrical panel behind the back door: The power to an electrical panel was not de-energized before an employee was assigned to cover the panel box.  Employee was exposed to electrical hazards.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  First floor, Large front room: The black power cord for temporary lights was pulled away from its insulation next to the junction box.  Employees were exposed to electrical hazards.
