29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 19, 2022, the employer had five employees working at 1360 West Brickstone Court, Appleton, Wisconsin, performing re-roofing activities on a residential roof with eave heights ranging from 10.5 feet to 25 feet and roof pitches from 6/12 to 9/12, without utilizing fall protection. Roof Damage Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1576932, citation number 1, item number 1a and was affirmed as a final order on March 15, 2022, with respect to a workplace located at 108 W 3rd Ave, Jerky Shore, Gulf Shores, Alabama 36542.
