29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about October 21, 2022, employees operating pneumatic nail guns without  eye or face protection while installing shingles,  were exposed to struck by hazards that cause puncture wounds and potential blindness or eye injury.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about October 21, 2022, employees conducting roofing activities (shingle installation) on a 4:12 pitch roof of the main house structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eighteen (18) feet on the South  side of the structure.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about October 21, 2022, the two portable extension ladders being used to access the roof of the structure did not extend at least three feet above the upper landing area. This exposed employees accessing and egressing to and from the roof to a fall hazard of approximately 10 feet.
